What is the PWD ID?
The PWD Identification Card is an official government-issued ID card that helps persons with disabilities access benefits, discounts, and government support. It is for Mandaluyong residents who have visible or non-apparent disabilities.
Who Can Apply?
Any Mandaluyong resident with a disability.
- Physical disability
- Visual impairment
- Hearing impairment
- Intellectual disability
- Mental or psychosocial disability
- Learning disability
- Non-apparent or hidden disability
Where to Apply
Requirements
Prepare the following items before going to the PDAD office.
- Medical certificate or clinical abstract na nagsasaad ng disability
- Three (3) pieces of 2Ă—2 ID pictures
- Two (2) pieces of 1Ă—1 ID pictures
- Blood type result
- Voter’s ID or Voter’s Registration
Note:
- If the applicant is a minor. Parent’s or guardian’s voter documents are accepted
- If you do not have ID pictures or blood typing. The City Government can assist you for free

Submit your requirements

Fill out the PWD Application Form
A staff member will help if you need assistance with writing.
Processing
The office will enter your details. prepare your ID. and create your booklets.
If you have no blood type result or need help with voter
registration.
PDAD will assist you.
Claim your PWD ID and booklets
You will receive your:
- PWD ID
- Medicines booklet
- Prime-commodities booklet
Staff will guide you on how to use them.
Fees and Processing Time
- The service is free
- Processing time usually takes around 30 minutes if your requirements are complete
After You Receive Your PWD ID
- Keep your ID and booklets with you
- Present them when using your benefits and discounts
- Use your card for transportation, medicines, and essential goods

Important Reminders
- The Mandaluyong City government has released a new version of the PWD ID with a QR code. Existing PWDs are encouraged to replace their old IDs.
